 Appointments are only scheduled online (see below), please click on the purple tab for new patient appointment scheduling or the yellow tab for follow up appointment scheduling.

Email [email protected] for any inquiries or questions prior to scheduling an appointment. Emails are returned within 24 hours or sooner. In case of emergency, please call 911 and do not call our office.

APPOINTMENT RESCHEDULING: If you need to move your appointment date and time simply look at the schedule below and email us a request to switch to another open time as long it's before 24hrs prior to appt time and date.

CANCELLATION POLICY: You can request to cancel an appointment up until 24hrs before your scheduled appointment time and have the fee paid refunded to your original method of payment minus $85 processing charge for new patient appointments and $40 processing charge for follow up appointments.
